Activity Description

The objective of these interprofessional rounds is to highlight challenging clinical cases while enhancing collaboration, team-based care, and outcomes for members with diabetes.

  1. Apply best-practice diabetes clinical care recommendations to improve member outcomes

  3. Assess factors that influence member health outcomes and identify strategies to support members in gaining access to care and achieving improved health
