Activity Description

The objective of this interprofessional series is to provide a collaborative forum to address current best practices for pharmacologic and non-pharmacologic strategies for care of chronic conditions. This series aims to integrate disease state management principles with current approaches to medication adherence, while bridging cross-functional approaches to gaps in care in pharmacology.
