This training equips Outpatient Behavioral Health providers with best practices for evidence‑based treatment planning and documentation by applying Medical Necessity Criteria (MNC), using SMART goals to connect documentation with measurable member progress, and integrating discharge planning throughout treatment to support continuity of care.

- Identify evidence-based practices used when treatment planning
- Explain best practices for treatment planning and documentation
- Describe Medical Necessity Criteria (MNC) and the importance of reflecting MNC in the clinical documentation
- Describe SMART Goal format and discuss how SMART format helps connect documentation, MNC and measurement of member progress.
