2026 Medicare Model of Care

Activity Description

The course provides an overview of the Medicare Model of Care (MOC), a CMS-required framework for Special Needs Plans.  It explains how the MOC guides care management, supports quality improvement, and ensures compliance through NCQA approval and CMS audits.

Centene Staff must take this course (PHCO 2026 Medicare Model of Care) through Workday Learning for credit.

This course is not yet eligible for Providers. 

Learning Objectives

  1. Outline the basic elements of Centene Model of Care (MOC).
  2. Explain how Centene Medical Management staff coordinate care for Special Needs Plan (SNP) members.
  3. Describe the essential role of providers in the implementation of the MOC program.
  4. Define the key quality and performance measures related to the MOC.
