Exploring Advance Care Planning (ACP)

The purpose of this training is to empower those who work with or support adults with the essential knowledge, communication strategies, and practical tools needed to effectively guide and support advance care planning.

Target Audience

Unlicensed and licensed LTSS professionals and stakeholders including, but not limited to, direct care workers, administrators, facility staff, home health staff, nursing aides. Unlicensed and licensed behavioral health professionals including, but not limited to, social workers, graduate-level counselors, therapists, and substance use professionals. Unlicensed and licensed health care professionals including, but not limited to, nurses.

Practice Level: Beginner and/or Intermediate.

Learning Objectives

  • Explain the purpose of advance care planning. 
  • Describe at least three types of advance directives. 
  • Identify individuals who would benefit from having an advance care plan in place. 
  • Recommend at least two key considerations when engaging in conversations about future care preferences. 
  • Write down one action caregivers can take to begin advance care planning.
