Inpatient Clinical Documentation MNC, Discharge Planning, and Readmission

This training equips Inpatient Behavioral Health providers with the knowledge and skills to apply Medical Necessity Criteria (MNC) in clinical documentation, initiate and sustain effective discharge planning throughout treatment, ensure timely 7‑day follow‑up, accurately complete discharge documentation, and use discharge planning strategies to help reduce hospital readmissions.

Target Audience

Mississippi Inpatient Psychiatric Hospital providers.

Licensed behavioral health professionals including, but not limited to, social workers, graduate-level counselors, therapists, and substance use professionals. Licensed health care professionals including, but not limited to, nurses.

Practice Level: Beginner and/or Intermediate.

Learning Objectives

  • Describe Medical Necessity Criteria (MNC) and the importance of reflecting MNC in the clinical documentation
  • Discuss when discharge planning starts and why it is important to address throughout treatment
  • Discuss importance of 7 day follow up
  • Identify components of Discharge Documentation
  • Discuss what role discharge planning plays in ways to help prevent readmissions
