LHCC presents Person Centered Planning: The 4Ps

The purpose of this training is to help service providers learn skills to ensure that the planning and subsequent service delivery for each person they serve is driven by the hopes, dreams, aspirations, and wishes of the person receiving the help. This is called person-centered planning.​

Person-centered planning is a collaborative process where people using behavioral health services participate in the development of service goals and in the selection of services provided, to the greatest extent possible. Person-centered planning is strengths-based, and focuses on an individual’s abilities, preferences, and goals. Individuals using services and their significant others are core participants in the development of the goals of, and plans for, treatment.
