Titration of Services for Providers

This module focuses on Titrating Services and is the third of four modules in this series.
This training series will allow audiences to review the concepts of care coordination, treatment planning, titration of services and discharge planning. These concepts will be applied to a high-level, fictious member scenario where each series module will show hypothetical examples and how each concept- care coordination, treatment planning, titration of services and discharge planning can be achieved.

Target Audience

Unlicensed and Licensed individuals who work with members and are involved in the delivery of care; including but not limited to therapists, case managers and care coordinators.

Learning Objectives

• Define titration of services.
• Identify the importance of titrating services.
• Review best practices for titrating services.
• Identify potential barriers when titrating services.
